| dc.description.abstract | Extrahepatische Cholangiokarzinome (eCCA) werden meist in fortgeschrittenen, nicht resezierbaren Stadien diagnostiziert. Die Standardtherapie besteht aus systemischer Chemotherapie (SC) und der Behandlung der biliären Obstruktion durch Plastik- oder Metallstents. Daneben können die endobiliäre photodynamische Therapie (ePDT) und die endobiliäre Radiofrequenzablation (eRFA) lokal tumorreduzierend wirken. Direkte Vergleiche beider Verfahren in Kombination mit der systemischen Chemotherapie sowie Daten zur sequenziellen Anwendung beider lokalablativer Verfahren fehlen weitgehend. Soweit bekannt ist dies die erste Studie, die den Einsatz beider lokaler Verfahren in Kombination mit der systemischen Chemotherapie untersucht. Methoden: Es wurde eine retrospektive monozentrische Analyse von 63 Patientinnen und Patienten mit nicht resezierbarem eCCA durchgeführt, die zwischen 2010 und 2020 am Universitätsklinikum Bonn eine palliative Systemtherapie und mindestens ein endobiliäres Verfahren erhielten. Hierbei wurden drei Gruppen gebildet: ePDT + SC (n = 22), eRFA + SC (n = 28), ePDT + eRFA + SC (n = 13). Die primären Endpunkte waren Gesamtüberleben (OS) und Verträglichkeit, zusätzlich wurden progressionsfreies Überleben (PFS), Ansprechraten sowie prognostische Faktoren analysiert. Ergebnisse: Das mediane OS betrug 12,7 Monate (ePDT + SC), 13,8 Monate (eRFA + SC) und 20,2 Monate (Triple-Therapie). Der Unterschied war jedoch nicht signifikant (p = 0,112). Unabhängige signifikante Prädiktoren für ein längeres Überleben waren eRFA (HR 0,44), mehr als eine Chemotherapie-Linie (HR 0,48) und die Implantation von Metallstents (HR 0,33). In Subgruppenanalysen wurden je nach Stadium bzw. Lokalisation des Tumors weitere therapieabhängige Überlebensvorteile gefunden. Schlussfolgerung: ePDT und eRFA sind in Kombination mit systemischer Chemotherapie bei fortgeschrittenem eCCA durchführbar und scheinen das Überleben positiv zu beeinflussen. Die Triple-Therapie zeigte den höchsten medianen Überlebenswert und deutet auf mögliche synergistische Effekte hin. | de |
| dc.description.abstract | Extrahepatic cholangiocarcinoma (eCCA) is usually diagnosed at an advanced, unresectable stage. Standard treatment consists of systemic chemotherapy (SC) combined with biliary drainage using plastic or metal stents. Endobiliary photodynamic therapy (ePDT) and endobiliary radiofrequency ablation (eRFA) can provide additional local tumor control. Direct comparisons of the two ablative techniques in combination with systemic chemotherapy, as well as data on their sequential use, remain scarce. To our knowledge, this is the first study to evaluate both local modalities together with systemic chemotherapy. Methods: In this retrospective single-center analysis, 63 patients with unresectable eCCA treated at the University Hospital Bonn between 2010 and 2020 received palliative systemic therapy plus at least one endobiliary ablation procedure. Patients were assigned to three groups: ePDT + SC (n = 22), eRFA + SC (n = 28), and ePDT + eRFA + SC (n = 13). Primary endpoints were overall survival (OS) and safety; progression-free survival (PFS), response rates, and prognostic factors were also assessed. Results: Median OS was 12.7 months with ePDT + SC, 13.8 months with eRFA + SC, and 20.2 months with the triple combination (p = 0.112). Independent predictors of longer survival were treatment with eRFA (HR 0.44), receipt of more than one line of chemotherapy (HR 0.48), and placement of metal stents (HR 0.33). Subgroup analyses indicated stage- and location-dependent survival benefits for the respective ablative modalities. Conclusion: Both ePDT and eRFA are feasible in combination with systemic chemotherapy for advanced eCCA and appear to confer a survival benefit. The triple combination achieved the longest median OS and may point to synergistic effects of the two local therapies. | en |
