dc.description.abstract | Bisher konnte bei Patienten mit Vorhofflimmern (VHF) in der CT-Angiographie der Koronar-arterien (CTCA) trotz EKG-Triggerung keine adäquate Bildqualität erzielt werden. Ziel der Arbeit war es, die Durchführbarkeit der CTCA bei Vorhofflimmerern unter Verwendung neuer frequenzadaptierter Algorithmen zu beurteilen sowie das optimale Rekonstruktionsintervall für diese Darstellung der Koronararterien bei Vorhofflimmern zu ermitteln und somit eine relevante Koronare Herzerkrankung (KHK) als Ursache des Vorhofflimmerns nicht-invasiv sicher auszuschließen. 20 Patienten mit permanentem VHF wurden an einem 16-Zeilen-Mehrschicht-Computer-tomographen untersucht (Kollimation: 16 mm × 0,75 mm; Rotationszeit: 0,42 s; 140 kV; 380 mAs; 120ml Ultravist 370® i. v.). Die Herzfrequenz (HF) der Patienten während der Untersuchung lag zwischen 42 und 156 Schlägen pro Minute (S/min); mittlere HF 78 ± 23 S/min. Jedes Koronarsegment wurde in 10 %-Schritten innerhalb des RR-Intervalls rekonstruiert. Die Beurteilung der jeweiligen Rekonstruktion beinhaltete: 1. die Bildqualität (5-Punkte-Skala) und 2. das Vorliegen von Diameterstenosen der Koronararterien (5-Punkte-Skala). Korreliert wurde die Stenosegraduierung mit der Katheterkoronarangiographie. Die beste Bildqualität und der sicherste Stenosenachweis (Sensitivität: 93,9 % und Spezifität: 70,8 %) fand sich für den Rekonstruktionszeitpunkt bei 40 % des RR-Intervalls. Dieses end-systolische Rekonstruktionsintervall zeigte sich dem diastolischen Standardrekonstruktions-intervall von 80 % (Sensitivität: 64 %, Spezifität: 59 %) signifikant überlegen (p < 0,01). Die zweitbeste Bildqualität wurde bei 0 % des RR-Intervalls ermittelt. Ein frequenzangepasster Algorithmus und die Wahl eines endsystolischen Rekonstruktions-intervalls ermöglichen es unter Verwendung der Mehrsegmentrekonstruktion, auch bei Patienten mit VHF in der Mehrschicht-Spiral-Computertomographie (MSCT) eine diagnostisch ausreichende Bildqualität zu erzielen und somit eine relevante Koronare Herzerkrankung (KHK) als Ursache des Vorhofflimmerns nicht-invasiv sicher auszuschließen. | |
dc.description.abstract | Multislice Spiral Computed Tomography of the Coronary Arteries in Patients with Atrial Fibrillation Purpose: As yet satisfactory visualization of the coronary arteries with multi-slice spiral CT angiography (MSCTA) in patients with atrial fibrillation (AF) could not be provided despite of retrospective ECG gating. The objective of this study was the evaluation of the application of MSCTA in patients with AF using a recently developed new algorithm and the estimation of the optimal reconstruction window for a reliable non-invasive exclusion of a relevant coronary artery disease (CAD) as cause for the atrial fibrillation. Materials and Methods: 20 patients with permanent AF were imaged on a 16-slice scanner (slice collimation: 16 mm x 0.75 mm; rotation time: 0.42 s; 140 kV; 380 mAs; 120 ml Ultravist 370® IV). The heart rate during the examination ranged between 42 and 156 beats per minute; the average heart rate was 78 ± 23. The patients had not received any previous drugs for heart frequency regulation. Acquisition was started after bolus tracking of a biphasic bolus of 120 ml Ultravist 370® injected intravenously. Each coronary segment was reconstructed at 0% - 90% of the cardiac cycle (RR-interval) in increments of 10%. The reconstructions were assessed by two blinded independent readers. The evaluation of each reconstruction included: 1. the quality of the visualization (five-point rating scale: 1 = very poor, 2 = poor, 3 = fair, 4 = good, 5 = excellent) and 2. the degree of stenosis (five-point rating scale: 1 = 0%, 2 = 1% - 49%, 3 = 50% - 74%, 4 = 75% - 99%, 5 = 100%). The degree of stenosis was correlated with the catheter coronary angiography. Results: The best visualization of all coronary artery segments and the most reliable detection of stenosis (sensitivity: 93.9 % and specitivity: 70.8 %) was generated at 40 % of the RR-intervall (mean score of the image quality: 2.79). This endsystolic reconstruction window was significantly superior (p < 0,01) to the standard diastolic reconstruction window at 80% (sensitivity: 64 %, specitivity: 59%) (mean score: 2.33). The second best image quality (mean score: 2.57) was acquired at 0% of the cardiac cycle. Conclusions: A frequency adapted delay algorithm in combination with an endsystolic reconstruction window, enables diagnostically valuable images with multi-slice spiral CT angiography (MSCTA) using multi-segment reconstruction even in patients with atrial fibrillation (AF). As a result a reliable non-invasive exclusion of a relevant coronary artery disease (CAD) as cause for the atrial fibrillation is possible. | |
