dc.description.abstract | Zur Therapie der portalen Hypertension und ihrer Komplikationen kann ein transjugulärer intrahepatischer portosystemischer Shunt (TIPS) implantiert werden. Dabei stellt die TIPS Dysfunktion ein klinisch relevantes Problem dar. Diese Dissertation untersucht den prognostischen Wert der TIPS Geometrie und Positionierung für die Entwicklung einer Dysfunktion in der zweidimensionalen Bildgebung. 330 Patienten, die zwischen 2014 und 2019 einen TIPS erhielten, wurden in dieser retrospektiven monozentrischen Studie analysiert. Dabei wurden neben klinischen Daten auch DSA Angiogramme der TIPS Implantation bezüglich Parameter wie Stent Positionierung und Geometrie untersucht. Der primäre Endpunkt war die Entwicklung einer TIPS Dysfunktion, die eine interventionelle Revision notwendig machte. Es wurde eine Dysfunktions-freie Gruppe aus 237 Patienten mit einer Dysfunktions-Gruppe aus 70 Patienten verglichen. Die Embolisation von spontanen portosystemischen Shunts (p = 0,006, HR 0,319, 95% CI 0,140-0,725) sowie die Position des kranialen Stentendes (p = 0,027, HR 2,300, 95% CI 1,101-4,806) waren signifikant mit der Entwicklung einer TIPS-Dysfunktion verbunden und konnten als unabhängige Prädiktoren für die Entwicklung einer TIPS Dysfunktion bestätigt werden. Als Schlussfolgerung dieser Studie sollte der Abstand des kranialen Stenendes zur Vena cava inferior bei der TIPS Implantation in der DSA weniger als 1 cm betragen, um das Risiko der Entstehung einer Dysfunktion zu senken. | de |
dc.description.abstract | Importance of the position and geometry of the transjugular intrahepatic portosystemic shunt for the outcome of decompensated liver cirrhosis A transjugular intrahepatic portosystemic shunt (TIPS) can be implanted to treat portal hypertension and its complications. TIPS dysfunction is a clinically relevant problem. This dissertation investigates the prognostic value of TIPS geometry and positioning for the development of dysfunction on two-dimensional imaging. 330 patients who received a TIPS between 2014 and 2019 were analysed in this retrospective monocentric study. In addition to clinical data, DSA angiograms of TIPS implantation were examined with regard to parameters such as stent positioning and geometry. The primary endpoint was the development of TIPS dysfunction requiring interventional revision. A dysfunction-free group of 237 patients was compared with a dysfunction group of 70 patients. Embolisation of spontaneous portosystemic shunts (p = 0.006, OR = 0.319, 95% CI = 0.140 - 0.725) and position of the cranial stent end (p = 0.027, OR 2.300, 95% CI 1.101-4.806) were significantly associated with the development of TIPS dysfunction and could be confirmed as independent predictors for the development of TIPS dysfunction. As a conclusion of this study, the distance of the cranial stent end to the inferior vena cava should be less than 1 cm during TIPS implantation in DSA to reduce the risk of developing dysfunction. | de |
