dc.description.abstract | Anorexia Nervosa (AN) ist eine chronische Essstörung, die hauptsächlich bei weiblichen Jugendlichen und jungen Frauen auftritt. Eine der gravierendsten Komplikationen im Langzeitverlauf ist eine Abnahme der Knochendichte, durch die es bereits in jungen Jahren zu Spontanfrakturen kommen kann. Bisher gibt es keine effektiven Behandlungsmöglichkeiten, um diesem Knochenmasse- und -dichteverlust bei Magersucht entgegenzuwirken. Das Ziel der vorliegenden Studie war es, zu überprüfen, ob eine Ernährungstherapie, die zu einer Gewichtszunahme führt, die Knochenstoffwechselaktivität von magersüchtigen Patientinnen im Vergleich zu einer gesunden Kontrollgruppe verbessern kann. Dazu untersuchten wir 19 Patientinnen mit AN (Alter 14,1 ± 1,4 Jahre, BMI 14,2 ± 1,4 kg/m2) sowie 19 gesunde Jugendliche gleichen Alters über den Zeitraum von einem Jahr. Innerhalb des Studienjahres wurden Blutabnahmen zu Beginn sowie zu acht weiteren definierten Zeitpunkten durchgeführt. Die Aktivität der Knochenformation wurde durch die Parameter knochenspezifische alkalische Phosphatase (bAP), C-terminales Prokollagen Propeptid (PICP) und Insulin-like Growth Factor (IGF-I) bestimmt. Die Knochenresorption wurde durch das C-terminale Telopeptid (CTX) nachgewiesen. Zusätzlich wurde das Blut auf Leptin, 17ß-Östradiol, Cortisol, DHEAS, PTH, 25-OH Vitamin D, Calcium, Phosphor und Kreatinin analysiert. Die Nährstoffaufnahme wurde anhand von Ernährungsprotokollen, die Körperzusammensetzung mit Hilfe der Bioelektrischen Impedanzanalyse bestimmt. Im Vergleich der beiden Gruppen zeigten sich zu Beginn der Studie Unterschiede in den Knochenformationsmarkern PICP (p < 0,001), bAP (p = 0,0004) und IGF-I (p < 0,0001). Zusätzlich waren in der Patientinnengruppe der BMI (p < 0,0001), Leptin (p < 0,0001) und 17ß-Östradiol (p = 0,0007) vermindert. Für die Magermasse wurden ebenfalls reduzierte Werte bei den Magersüchtigen festgestellt (p = 0,02). Nach einer 15-wöchigen Ernährungstherapie erhöhten sich die Konzentrationen der Knochenformationsmarker der Patientinnen und glichen sich an die der Kontrollgruppe an. Zum Ende der Studie sanken die Werte von IGF-I und Leptin erneut ab (p < 0,001). Im Vergleich zur Kontrolle blieben die Fett- sowie die Körperzellmasse über den gesamten Studienzeitraum vermindert (p < 0,001). Folgende Schlussfolgerungen leiten sich aus diesen Ergebnissen ab:

dc.description.abstract | Influence of a nutritional therapy on bone metabolism in young anorectic patients Anorexia Nervosa (AN) is a common chronic eating disorder in female adolescents and young women. The restrictive eating pattern is associated with profound metabolic complications. One of the most important long-term somatic problems is the decrease in bone mineral density, which often leads to spontaneous fractures later in life. To date there is no effective treatment for this bone loss in Anorexia Nervosa. The aim of the study was to investigate whether a nutritional therapy leading to a weight gain improved bone metabolism in patients with AN compared to a healthy control group. We studied 19 female patients with AN (age 14.1 ± 1.4 years, BMI 14.2 ± 1.4 kg/m2) and 19 healthy adolescents (age 15.1 ± 2.3 years, BMI 20.8 ± 1.9 kg/m2) for one year. Blood was drawn at baseline and at eight defined points of time (five times for the control group) in the following year. Bone formation activity was measured by the parameters bone alkaline phosphatase (bAP), C-terminal procollagen-I-propeptide (PICP) and insulin-like growth factor (IGF-I). Bone resorption was determined by the C-terminal telopeptide (CTX). In addition we analyzed blood for leptin, 17ß-estradiol, cortisol, DHEAS, PTH, 25-OH-vitamin D, calcium, phosphor and creatinine, respectively. In parallel urine was sampled on three to seven consecutive days around every blood draw to determine volume and calcium excretion. Nutrient intake was estimated from food protocols. Metabolic calcium balance was calculated from the respective intake and excretion data (urine and faeces). Body composition was estimated by means of bioelectrical impedance analysis. Measurements of bone density were taken at baseline, and after four and eight months. Comparing the two groups with a repeated–measures-ANOVA adjusted for age and menarche status we found different baseline values for the bone formation markers PICP (p < 0.0001), bAP (p = 0.0004) and IGF-I (p < 0.0001), respectively. In addition BMI (p < 0.0001), leptin (p < 0.0001) and 17ß-estradiol (p = 0.0007) were significantly lower in the patient group. Lean body mass was also reduced in the patients (p = 0.02).After 15 weeks of dietary treatment there was an adjustment of bone formation activity between both groups.At the end of the study levels of IGF-I and leptin decreased again (p < 0.001). In comparison to the healthy subjects, the patient group showed reduced values for fat mass and body cell mass during the whole study.There were no distinctions between patients and controls in bone resorption activity, DHEAS, calcium, phosphor and bone density throughout the entire study.
