dc.description.abstract | Ziel der Studie war es, Risikogruppen von Patienten herauszustellen, die nach Darmresektion eine intestinale Hyperoxalurie aufweisen, die in der Folge zu einer Bildung von Calciumoxalat-Harnsteinen führt. Hierzu haben wir eine Gruppe von 6 Patienten mit Resektionen von 40-50cm terminalem Ileum unter Belassung der Bauhinschen Klappe zur Formation einer orthotopen Ileum-Neoblase nach Harnblasencarcinom einer Gruppe von 9 Morbus Crohn-Patienten mit Resektionen von 40-60cm terminalen Ileum und z.T. auch Colonresektionen, sowie der Bauhinschen Klappe gegenübergestellt. Als Vergleichskollektiv diente eine Gruppe von 12 gesunden Probanden. Zur Bestimmung der intestinalen Hyperoxalurie wurde der [13C2]Oxalatabsorptions-test angewandt. Unsere Ergebnisse zeigen, dass die Werte in der Gruppe der Ileumneoblase-Patienten mit Oxalatabsorptionen von 5,1-12,4% gut mit den Werten der gesunden Probanden übereinstimmen, während die Morbus Crohn-Patienten fast ausnahmslos eine Hyperoxalurie aufwiesen. Ebenfalls litten die Crohn-Patienten allesamt unter postoperativen Komplikationen, wie Diarrhoe und Harnsteinbildung, welche bei den Neoblase-Patienten nicht zu verzeichnen waren. Wir kommen in unserer Studie zu dem Ergebnis, dass Patienten mit Darmresektion zur Anlage einer orthotopen Ileumneoblase keine Gefahr laufen, eine intestinale Hyperoxalurie und folglich eine Calciumoxalatnephrolithiasis zu entwickeln. Den entscheidenden Punkt stellt hierbei die Operationsmethode dar, bei der die letzen 20-30cm terminalem Ileums inclusive der Bauhinschen Klappe belassen werden, was wiederum einen positiven Effekt in Hinsicht auf Transitzeiten des Speisebreis und das Resorptionsverhalten des verbliebenen Ileums hat. Aufgrund der niedrigen Fallzahlen sollten weitere, umfassendere Studien folgen, um unsere These zu stützen. | |
dc.description.abstract | Intestinal oxalate absorption evaluated by the [13C2]Oxalate absorption test in patients with orthotopic ileal neobladder in comparison to Morbus Crohn-Patients after resection of ileum and colon The objective of the study is to line out groups of patients running risk to develop intestinal hyperoxaluria and consequently calciumoxalatenephrolithiasis after gastrointestinal surgery such as resection of ileum and colon. Therefore we examined a group of 6 patients with resection of 40-50cm of terminal ileum excluding the iliocoecal valve in order to form an orthotopic ileal neobladder after urothelcarcinoma as well as a group of 9 Crohn-patients with resection of 40-60cm of terminal ileum including the valve as well as colon. The reference range of intestinal oxalateabsorption was measured in 12 healthy volunteers. The function test applied was the [13C2]oxalate absorption test. With a range of 5,1-12,4% the oxalate absorption of the neobladder-patients is well within the reference range of the healthy volunteers whereas all of the Crohn-patients presented a hyperoxaluria. Furthermore all the Crohn-patients suffered from postoperative complications such as diarrhea and nephrolithiasis which could not be found among the patients with orthotopic neobladder. We come to the conclusion that patients with such continent urinary diversion present no hazard for oxalate hyperabsorption and subsequent oxalate urolithiasis. The most relevant explanation for this fact lies in the surgical technique where 20-30cm of the most terminal ileum including the ileocoecal valve are retained which has a positive impact on the transit of digested meals and the absorbative function of the remaining ileum. Due to the small number of test-participants larger studies should be performed to support our thesis. | |
